This code is used to report the first 30 to 74 minutes of critical care given on the same date.  This code can be used ONLY ONCE per calendar date.  Time spent DOES NOT need to be continuous.  Time MUST be documented in the chart.  The physician must spend over 30 minutes in total critical care time in any ONE calendar date to use the 99291.  Time spent of less than 30 minutes (even if the care is critical) is reported using the normal hospital E/M codes.

Internists used this code 672,812 times in 2003.  In that same year, the 99291 was reported 126,858 times by family practitioners, 52,129 times by nephrologists, 163,062 times by cardiologists and 482,526 times by pulmonologists.  The reimbursement for the 99291 is approximately $239.70.
