Level 2 H&P (99222)
The 99222 represents the second highest level of initial care for patients being admitted to the hospital. This is the second most popular code used to bill for admission H&Ps among internists who select the 99222 for 28% of these encounters. The reimbursement for this level of care is approximately $132. Usually the presenting problems are of moderate severity.
The documentation for this code requires THREE out of THREE of the following :
1) Comprehensive History
2) Comprehensive Exam
3) Moderate Complexity Medical Decision-Making
Or 50 minutes spent face-to-face
with the patient if coding based on time. The appropriate
documentation must be included.
