This video tutorial is designed for residents who are new to rules of E/M coding and documentation. Using real clinical examples, the fundamentals of the key components of history, exam and medical decision-making are explained. Then residents learn how to perform the documentation in a purpose-driven manner to ensure that the history and exam are congruent with the level of care selected. Finally, the rules for "tethering" the documenttaion to the attending's note are discussed.
