This course explains the nuts and bolts of coding and documenting encounters using the 1995 and 1997 E/M guidelines, which are still operative for hospital-based E/M visits. We review the key components of history, physical exam and medical decision-making, which form the building blocks of the documentation when using the 1995 or 1997 rules to record your patient encounters.

The module applies the principles of rational physician coding to nursing facility E/M services.  Learn how to select the "correct" level of care for these encounters, then see how to construct the history and exam in a purpose-driven manner to make sure these key components are congruent with the code selected.
