Straightforward Complexity MDM

Straightforward Complexity MDM is the lowest level of MDM. To qualify for SF complexity MDM the requirements for two out of the three dimensions of MDM listed below must be satisfied.

Problems addressed: one self-limited or minor problem.
Data reviewed: minimal or no data reviewed.
Risk: minimal risk or morbidity from additional diagnostic testing or treatment

When coding based on the MDM, straightforward complexity MDM is required for:

level 2 established office visits (99212)
level 2 new office visits (99202)
level 1 admission H&Ps (99221)
level 1 hospital progress notes (99231)
level 2 outpatient or inpatient consults (99242, 99252)
level 2 ER visits (99282)
The lowest level of care for initial nursing facility care visits (99204)
The lowest level of care for subsequent nursing facility care visits (99307)
